Do you see blood in your urine? Do you are feeling extremely serious discomfort in your stomach, groin or sides? If your answer’s yes, then see your physician because you could have kidney stones.
Research has demonstrated that one in each twenty individuals develop kidney stone ( s ) at some point in their lives. The surgeon takes away the stone ( s ) by cutting the side or belly of the patient so as to reach the kidneys. The surgeon places a narrow telescope into the patients kidney thru an incision in the back of the patient.
The surgeon then takes away the stone ( lithotomy ) or breaks it up and then takes away the split up stones ( lithotripsy ). You will feel fine and show no outward symptoms, but damage is slowly amassing within your body. Reducing blood pressure levels straight away and maintaining healthy numbers is the key to being able to avoid them. Kidney Failure – Because they filter and clean the blood and are stuffed with small arteries, the kidneys are very exposed to hypertension. Kidney failure is a leading reason for untimely death among raised blood pressure sufferers and diabetes patients.
Coronary coronary disease – When buildup forms on the veins leading into the heart and blood flow is reduced, it is commonly known as coronary coronary disease. When these blockages build to a point where blood flow is seriously impeded, it ends up in a cardiac arrest.
* homemade treatments for kidney stones Most renal calculi that are about 5mm to 10mm customarily pass all alone because research has proven that 1 or 2 out of each 10 kidney stones need medical aid. It takes almost one to 3 weeks for renal calculi to pass after using home cures.
* Vitamin additions : Taking vitamin An and B6 supplement will certainly stop the formation of renal calculi and treat kidney stones permanently, that is, if it is taken for a few months. * Pomegranates : Seeds of pomegranates ( both sweet and sour ) have proved useful in treating renal calculi.

I've passed 3 kidney stones. I've been told by both Doctors and women who have experienced childbirth and kidney stones that the kidney stones are worse. So I've concluded that the whole “pain of childbirth” thing is just another something to bitch about and tell us we can't understand. Like high heels, getting all pretty in the morning, menstruation, menopause, and laundry.

Well done and all the best for the future!
I love to hear of success stories like yours. I am witnessing so many colleague`s lives around me being destroyed, some already have gone because of the cocktail of drugs they are prescribed in connection with high blood pressure.
The more people speak out about trying and believing in alternative ways of reducing blood pressure, the more others will have the confidence to take that step too.
If people can try and change lifestyle, diet, reduce stress, and take the right supplements to support their bodies then the majority would regain health and the drug companies would loose their vice-like grip.

In regards to Stephanie saying that her doctor recommended 1 or 2 glasses of wine daily for kidney stones…get a new doctor….according to Mayo Clinic which is an authority on kidney stones…For people with a history of kidney stones, doctors usually recommend passing at least 2.5 quarts (2.3 liters) of urine a day. To do this, you'll need to drink about 14 cups (3.3 liters) of fluids every day — and even more if you live in a hot, dry climate.
What should you drink? Water is best. Include a glass of lemonade every day, too. Make your own with real lemons, or use a liquid or frozen concentrate, but avoid powdered lemonade mixes. Lemonade increases the levels of citrate in your urine, and citrate helps prevent stone formation.

A bit of graffiti found near Harrow Road, alongside the Grand Union Canal. Someone has gone to a lot of trouble for a very weak punchline. It read:
“Once upon a time there was a Bear and a Bee who lived in a wood and were the best of friends. All summer long, the Bee collected nectar from morning to night while the Bear lay on his back basking in the long grass.
When Winter came, the Bear realised he had nothing to eat and thought to himself “I hope that busy little Bee will share some of his honey with me”. But the Bee was nowhere to be found – he had died of a stress induced coronary disease.”
I guess there’s a lesson in that for all of us.
During past few years of adversity, I have had plenty time to read Warren Buffet's Essays.
Like majority of his followers, I could not fail to notice to sacredness of his investment style in terms of principles of money.
However, one stark fact that struck after few years of following him, is that Warren Buffet, goal of investment is solely for the purpose of generating long term returns. In money principles, his mantra is faultless.
But I differ from Warren Buffet in few respects:
1. His investment in Oil in China. He made his billions quickly in few years.
But then he is not like one of those investors who invest in oil exploration
or alternative energy solutions, because these investments do not have
“guaranteed returns”.
2. He sets monetary goals for his managers. After in depth research, I came to
conclusion, that the methods applied by his managers to accomplish monetary goals,
though legal, are not always ethical.
Even though in principle, I would love to follow Warren Buffet, I think there are far more venture capitalists around, who make risky investments in newer technologies, newer products, even newer hypothesis, that really make a difference ordinary peoples life. And I would love to see such newer investments see the light of the day.
As opposed to an investment in Coke which over the years could provide 100000% return as per Warren Buffet's investment strategies (which by the way over the decades has contributed so much to American obesity), I would prefer to invest in maybe a technology or research that could find solution to let us say Alzheimer, polio, diabetes, coronary disease, organ regeneration, and at extreme may be space exploration (private or NASA).
i have this concern sir.. why does the specific gravity remains in normal range?… knowing that you have this acute renal failure?
Well, don’t dwell on the possibity of a kidney stone. Just try to live a healthy lifestyle and get yearly doctor visits. Are you really worried about it?

Answers: (Sorry, was not on internet yesterday)
Ist radiograph:
Immediately apparent is the Dynamic Hip Screw on the right and a non-union or refracture of the right neck of femur (due to the 2nd fall).
Gross osteopenia/porosis is noted in the spine.
The bowels are dilated with faecoliths seen within.
And bilateral J-stents from the kidneys to the urinary bladder is noted.
There is no evidence of osteolytic lesions.
2nd radiograph:
It is hard to see # of the spine on AP view. A lateral xray lower thoracolumbar spine should be ordered.
As above, gross osteoporosis is noted in the spine.
The bowels are grossly dilated.
The J-stents mentioned earlier are also noted.
With his age of 64 years, high ESR, evidence of chronic renal failure and reversal of A:G ratio, one has to ask a few questions:
1. What was the cause of the renal failure? Is it calculi? (But there is no evidence of renal calculi on the xrays). Since the J-stents are inserted, one could assume the renal failure is most likely from an obstructive origin, in this case, perhaps ureteric stricture.
2. The high ESR and reversal of A:G ratio should prompt one to consider multiple myeloma, particularly in an elderly male. MM can cause hypercalcemia which can cause renal failure as well as dilated bowels (remember the moans, groans and psychic moans of hypercalcemia?). It can also result in pathological fracture.
3. Chronic renal failure may lead to osteoporosis by secondary or tertiary hyperparathyroidism. A serum iPTH level should be ordered.
To confirm the diagnosis of MM, one should:
1. order a calcium level and calculate the corrected Ca level to look for hypercalcemia.
2. Order urine Bence Jones protein (not always present)
3. A serum electrophoresis should be ordered to rule out monoclonal bands
4. Bone marrow trephine and biospy to look for plasma cells.
5. Since the patient complains of backache, a bone scan should be ordered.
Actually I didn't ask for management but most of you also mentioned the management of MM as well! Good!
The purpose of putting up this radiographs were to highlight:
1. Osteoporosis
2. A dynamic hip screw
3. Non-union/re-fracture of right neck of femur
4. Blateral J-stents
5. Dilated bowels.
A few comments:
1. Jing, those catheters are not for peritoneal dialysis
2. CK, Chee Koon, they are not pig tails either.
3. w1, 'referring to this or that person' is generally not seen as a good answer. In exams, you must assume you ARE the orthopedic surgeon or the heamatologist!
Kudos for this 'hard' case goes to: CK (when I read your answer, I also, like Chee Koon, went 'wow! ~ bear in mind, not all are correct but you got most of it), dytia, peanut, oyy and TFTD.
Thank you for taking part. I wish some junior students would attempt to answer also as those who answered (from IMU) are less than 365 days from becoming doctors!
